DO YOU TAKE MY INSURANCE?

 

We accept most insurances. For specific information regarding your benefits and coverage, either give us a call with your insurance information or fill out the request form below and our billing officer will get back to you within 24 hours to verify your benefits.

WHAT CAN I EXPECT FOR MY FIRST APPOINTMENT?

 

Please arrive 15 minutes early to find parking, locate our building, and fill out the intake form. If you are driving, you can locate 2 hour street parking in nearby adjacent streets. You can expect to be evaluated and treated on your first day. First day appointments usually last about 1 hour.

WHAT SHOULD I BRING TO MY APPOINTMENT?

 
  • Comfortable clothing that you can move in and exposes the area of concern (i.e. shorts if you have knee issues).

  • Insurance card and photo ID

  • Medical imaging (x-ray, MRI, etc) if applicable

  • Doctor referral (if applicable)

DO I NEED A DOCTOR REFERRAL?

 

In California, a patient can be seen directly by a physical therapy without a referral for 45 days or up to 12 visits, which ever comes first. Subsequent treatment will require a physician referral.

WHAT IS YOUR CANCELLATION POLICY?

 

We understand that life happens. We ask that you cancel/change your appointments at least 24 hours in advance by calling our San Francisco Physical Therapy Team or emailing us at info@somaphysio.com to avoid a $110 cancellation fee.
